EVV Support in office hours are Monday-Friday 9:00 pm - 5:00 pm.

The state requires us to have documentation for everything. Forms must have correct and complete information widthwise they will NOT get processed. Please look over the replied emails for PROCESSED or UNPROCESSED. UNPROCESSED forms will need to be resubmitted and could result in a delay or no pay. (El estado nos obliga a tener documentación para todo. Los formularios deben tener información correcta y completa a lo ancho, NO serán procesados. This form is for single occurrence only. You need to fill out and send in a form for each day/time you need to be updated. (Este formulario es solo para una sola ocurrencia. Debe completar y enviar un formulario para cada día/hora en que debe actualizarse.)

Example: If you are missing 3 days/times, you need to fill out and submit 3 forms. (Si faltan 3 días/horas, debe completar y enviar 3 formularies)

ADD - Missing hours. We can only add time to past dates or times. UPDATE - Time correction. We can only update the time you have already worked. DELETE - Remove the day and times, worked.
